A Sensor-Based Decision Support System for Transfemoral Socket Rectification. A decision support system (DSS) was created with the goal of enhancing the functionality of transfemoral prosthetic socket design and cutting down on the time required for the final socket design.
The DSS outputs suggestions for socket-rectification actions to the prosthetist. The DSS uses a fuzzy-logic inference engine (IE) for this purpose, which combines a set of rectification rules with pressure readings produced by socket-embedded sensors to determine the necessary rectification operations.
The latter is then handled by an algorithm that takes a 3D digital socket model and receives, edits, and outputs it as a triangle mesh formatted inside an STL file.
By modelling and comparing the wearing process between a good-fitting socket, a loose socket (poor fit), and numerous rectified sockets created by the suggested DSS, the DSS results were confirmed and tested in a FEA simulation environment.
The outcomes of the simulation show that decreasing the volume increases the pressure distribution over the stump. However, as volume reduction grows along with socket rectification intensity, uncomfortable high pressures start to occur in other areas of the socket.
As a result, a compromise must be made between the degree of rectification and the harmony of the pressure distributions felt at the stump.
The socket fitness exam depends on verbal feedback about the patient's comfort as well as visual observation of gait performance and skin irritation around the stump.
A theoretical framework, put forth by Radcliffe and other pioneers based on traditional engineering mechanics concepts and clinical data, governs the rectification activities, the socket shape alterations for enhancing fitness.
One major aspect limiting the degrees of understanding and expertise in prosthetic biomechanics is the absence of quantification. The qualitative technique lacks in-depth understanding of interface loading and calls for extremely strong clinical background to recognise the subtle signs of poor fitness.
